FOR INTERNATIONAL STUDENTS

Where to start as a new Host Family
Specific Information for International Students and Host Families
When you have been assigned a student, an information packet will be mailed to you.
Return the following forms from the packet:
- Student Information Sheet - including doctor's name and phone number and emergency contact information. Will be mailed to the host family to complete.
- Directory Authorization/ID Card Information – signed by host parent or guardian (if student resides with the guardian)
- Change of address/phone/email - every time a student moves to your home, original to the school office
- Address/Information Check – data updated regularly from this form, required by Dept of Homeland Security, host family signature indicating you have a copy of this information.
- Affidavit of Relationship for Homestay Purposes - for students living with blood relatives
Forms to be signed by the parent or guardian - one copy to school, one copy to host family before student begins classes
